29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a) During work at 29 Bentley Way; a safety program was not developed & implemented by the employer that addressed isolation of machinery and equipment, through lockout/tagout practices, to prevent injury of employee(s) due to unexpected start-up or release of stored energy.  Installation & service of gas fireplace(s) at customer sites exposes installer(s) and/or helper(s) to the hazard(s) of ignition of leaking or residual flammable gas in supply lines.    Condition(s) existed on and before 4/01/2015

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    a) During work at 29 Bentley Way in Colonia NJ; a written plan had not been developed & implemented for communicating information on the hazard(s) of chemical(s) in the workplace to which employee(s) have exposure.  Installer(s) & helper(s) have exposure to health hazards when they work with Portland cement as well as inhalation hazards of crystalline silica during demolition and re-facing of stone & brick frames of fireplaces at customer sites.    Condition(s) existed on and before 4/01/2015.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training as specified in 29 CFR 1910.1200(h)(2) and (3) on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard was introduced:   a) During work at 29 Bentley Way in Colonia NJ; employee(s) who are exposed to chemicals, such as but not limited to crystalline silica, when demolishing and resurfacing brick and stone framed fireplaces were not informed and trained on the requirements of the OSHA standard on chemical hazard communication.  Condition(s) existed on and before 4/01/2015.
