Blackhawk Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(a)(1), which was contained in OSHA inspection number 664919 citation number 1, item number 3b and was affirmed as a final order on November 6, 2012, with respect to a workplace located at 1201 West Jefferson Street, Springfield, IL 62704.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. Employees engaged in roofing activities on a low-sloped roof were exposed to a fall hazard of approximately 13ft 4in and none of the fall protection systems specified above were being used.
