Employees were allowed to jump into the Equipter drivable dumpster from the roof and the competent person did not recognize the fall hazards and prohibit the activity.  b. Employees who were assigned the task of fall protection monitor performed other roofing activities while on the roof and the competent person did not inspect the site for the hazard and prohibit the monitor from performing other work.
