Equipment includes, but is not limited to the following:       a. The  hair-pin guarding on the(company# GAK 400220) Bliss mechanical power press located in the press room was not adjusted to prevent entry of hands or fingers into the point of operation.       b. Company no. 400022: mechanical power press located in the press room had an opening in the guard which measured approximately 7.5-8 inches and did not prevent entry of hands or fingers into the point of operation.       c. The Niagara: mechanical power press (Co. #GEK #140330) that had A  and/or B gate guard did not prevent the operator from reaching over into the point-of-operation.
