29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: (a) Second floor top plate, new townhome construction located at 2388 Whiteoak Bend, Smyrna, GA 30080: Employees were exposed to overhead struck-by hazards while working below roof trusses which were hoisted directly overhead by a crane. Employees were not provided with protective helmets while working below the elevated load.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: (a) Second floor top plates, new townhome construction located at 2388 Whiteoak Bend, Smyrna, GA 30080: Employees operating Hitachi (Model NR83A3) Framing Nail Guns were exposed to eye injuries while operating these tools without protective eyewear. Employees operating these tools a 100 PSI were exposed to plastic debris, and 3-inch fasteners as safety glasses and or goggles were not worn.
