29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. On or about March 30, 2015 and at times prior thereto, an employee was located underneath of a wooden member being hoisted to the roof without wearing a helmet.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). At or about March 30, 2015 and at times prior thereto, employees were exposed to an approximate fall distance of 9 feet while working in the vicinity of an unguarded edge without any fall protection. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date completed, method of abatement, and states employees and their representatives were informed of this abatement.
