Big Bend Blvd.: Two employees who worked on a flat roof approximately 13 1/2 feet from the edge of the roof to ground level were not provided required training to recognize fall hazards and the necessary procedures to follow in reducing exposure to the hazards.

29 CFR 1926.1053(b)(1): The side rails of portable ladders used for access did not extend at least 3 feet above the upper landing surface, or was secured at its top to a rigid support that will not deflect and along with a grasping devise to assist employees mounting and dismounting the ladder: Murdoch & South Big Bend Blvd., Webster Groves: Employees used a Werner aluminum extension ladder with side rails extended 1 foot above the landing surface and was not secured otherwise to gain access to a flat roof which was approximately 13 1/2 feet to ground level.
