29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was 6 feet or more above a lower level was not protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system. (a) M&B Construction Services, Inc. at 3200 Pecos St, Denver, CO 80211: On or before March 26, 2015 the controlling employer did not ensure that subcontractor employees installing wood deck boards for benches on top of a one-level section of the building were protected from injury, in that two subcontractor employees worked near the edge without the use of a fall protective system. This condition exposed the workers to a fall of approximately 14 feet.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds were not fully planked or decked between the front uprights and the guardrail supports: (a) M&B Construction Services, Inc. at 3200 Pecos St, Denver, CO 80211: On or before March 26, 2015 the controlling employer did not ensure the working platforms on the fabricated frame scaffold were fully planked, in that a subcontractor employee was working on a section of the scaffold where only one plank was installed. This condition exposed the subcontractor employee to a fall of approximately 19 feet.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: (a) M&B Construction Services, Inc. at 3200 Pecos St, Denver, CO 80211: On or before March 26, 2015 the controlling employer did not ensure subcontractor employees working on a scaffold were protected from falls by the use of guardrails along all open sides and ends of the scaffold. End-rails were not installed on the top level of the scaffold where a subcontractor employee installed OSB onto the exterior of the building. This condition exposed the subcontractor employee to a fall of approximately 28 feet. (b) M&B Construction Services, Inc. at 3200 Pecos St, Denver, CO 80211: On or before March 26, 2015 the controlling employer did not ensure subcontractor employees working on a scaffold were protected from falls by the use of guardrails along all open sides and ends of the scaffold. End-rails were not installed on a mid-level section of the scaffold where a subcontractor employee installed OSB onto the exterior of the building. This condition exposed the subcontractor employee to a fall of approximately 19 feet.
