THE U.S. POSTAL SERVICE WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1910.303 (g)(1) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1020342, CITATION 1, ITEM 2 AND WAS AFFIRMED AS A FINAL ORDER ON APRIL 16, 2015 WITH RESPECT TO A WORKPLACE LOCATED AT 337 WEST CLARK STREET, EUREKA, CA 95501.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  Third Floor: On March 23, 2015 and at times prior thereto, the electrical power supply cord used to power the vacuum cleaners at location C6 and G3 had damaged insulation.  The outer sheathing insulation was pulled away from the wires and created a hazard to employees when used.  Third Floor: On March 23, 2015 and at times prior thereto, the electrical power supply cord used to power the vacuum cleaners at location H3 had missing grounding prongs.    THE U.S. POSTAL SERVICE WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1910.334 (a)(2)(i) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 990030, CITATION 1, ITEM 1 AND WAS AFFIRMED AS A FINAL ORDER ON DECEMBER 3, 2014 WITH RESPECT TO A WORKPLACE LOCATED AT 313 GRAND BLVD, VENICE, CA 92101.
