29 CFR 1910.215(b)(9):  The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  a)  Carpenter Shop, 5230 Centre Avenue, Pittsburgh, PA 15232:  Observed on or about March 20, 2015, a Dayton brand bench grinder (model number 2Z425S) was not equipped with a tongue guard.  The gap between the surface of the grinding wheel and the frame of the machine was approximately 9/16 inches.  Employees using the bench grinder were exposed to struck-by hazards and fire hazards due to the missing tongue guard.

Employees working in the shop were exposed to fire and electrical shock hazards created by the lack of access to the electrical panel.
