29 CFR 1910.151(c): Where the eyes or body of any person would have been exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    a) Firestone Complete Auto Care Building 245, Langley AFB, VA, Bay #4:  On or about 03/30/15 and times prior thereto, a wall-mounted emergency eye wash station (green in color with placards identifying it as an emergency eye wash station) did not contain any suitable liquid for immediate flushing of the eyes or body where corrosive chemicals were used in the work area such as battery acid, battery cleaners and protectants.    b) Firestone Complete Auto Care Building 245, Langley AFB, VA, Bay #1: On or about 03/30/15 and times prior thereto, a fixed emergency eye wash station was blocked by a tire stored in front of it and had a box of "Fog-Be-Gone" stored on top of it preventing potentially exposed employees from immediate emergency first aid use.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were connected to devices and fittings so that strain relief was not provided that would prevent pull from being directly transmitted to joints or terminal screws.    a) Firestone Complete Auto Care Building 245, Langley AFB, VA, Bay #4:  On or about 03/30/15 and times prior thereto, an automotive lift was observed with energized electrical cords containing twist lock style plugs coming from the electrical power unit on the side of the main lift support that had missing strain relief where the wire entered the plug end.    b) Firestone Complete Auto Care Building 245, Langley AFB, VA, Bay #4: On or about 03/30/15 and times prior thereto, an automotive lift was observed with energized electrical cords that had missing strain relief where the wire entered the electrical motor housing.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

29 CFR 1910.334(a)(1): Portable equipment was not handled in a manner which would not cause damage. Flexible cords may not be fastened with staples or, were hung in such a fashion as could cause damage to the outer jacket or insulation.    Firestone Complete Auto Care Building 245, Langley AFB, VA, between Bay #3 and Bay #4:  On or about 03/30/15 and times prior thereto, an extension cord (temporary wiring) was observed hanging in the overhead beside a manufactured cord reel.  The cord was "zip-tied" and in contact with bare metal ceiling/equipment supports with apparent unprotected sharp edges.  The male end of the electrical extension cord was plugged in to the overhead receptacle and the female end was plugged into a Northern Industrial portable 90 degree 7 inch Sander/Polisher containing a cupped style wire brush.     Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

29 CFR 1910.1200(f)(10): The employer did not ensure that workplace labels or other forms of warning were legible, in English, and prominently displayed on the container, or readily available in the work area throughout each work shift.    a) Firestone Complete Auto Care Building 245, Langley AFB, VA, Bay #1:  On or about 03/30/15 and times prior thereto, an unmarked and unlabeled milk-jug style plastic container was observed sitting on the floor containing an unknown purple color liquid.  It was later determined by the Store Manager that it was transmission fluid.  The containers content was almost approximately half full.    b) Firestone Complete Auto Care Building 245, Langley AFB, VA, Bay #1:  On or about 03/30/15 and times prior thereto, two additional unmarked and unlabeled milk-jug style plastic containers were observed on a plastic shelving unit located in Bay #1 right beside the shop supervisors desk.  They contained a liquid residue where they once had an unknown content in them.  The color content matched the same color as the one at contained transmission fluid.     Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

Only a generic corporate loss prevention policy for "Personal Protective Equipment" was provided, stating that PPE should be used "whenever and wherever it is necessary by reason of hazards in the work process or environment" but never identifies what those hazards are.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
