The employer did not provide a training program to enable employees to recognize the hazards of entrapment. This condition exposed the employees to entrapment and/or cave-in hazards

29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins and indications of failure of protective systems: Construction Site: On or about March 30, 2015 and at times prior thereto, four employees working between the excavation face and a retaining wall without any type of entrapment and/or cave-in equipment. The employer failed to have a competent person conduct a daily inspection. This condition exposed the employees to entrapment and/or cave-in hazards.
