29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) University Baptist Church: On March 30, 2015 an employee was not wearing head protection while performing clean up activities on the ground below other employees performing re-roofing activities above.   Note) Certification of corrective action IS required for this item.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) University Baptist Church: On March 30, 2015 employees performed roofing activities on a 5 in 12 pitched roof without a fall protection system in place.    b) University Baptist Church: On March 30, 2015 an employee worked at the edge of the 5 in 12 pitched roof cleaning out the gutters, without a fall protection system in place.          Note) Certification and documentation of corrective actions ARE required for this item.
