29 CFR 1910.132(a):  Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    (a) McDonalds Restaurants of Colorado, Inc., at 1905 S Federal Boulevard, Denver, CO: On 3/19/15 McDonald's Corporation did not ensure that protective equipment was used when necessary whenever hazards capable of causing injury and impairment were encountered.  One employee filtered approximately 250 degree Fahrenheit oil while cleaning the fryer.  The employee was not using a heat resistant apron while filtering the oil.  This condition exposed the employee to a burn hazard.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:     (a) McDonald's Restaurants of Colorado, Inc., at 1905 S Federal Boulevard, Denver, CO: On 3/19/15 McDonald's Corporation did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from liquid chemicals.  One employee filtered approximately 250 degree Fahrenheit oil while cleaning the fryer.  The employee was not using eye or face protection while filtering the oil.  This condition exposed the employee to a burn hazard.     (b) McDonald's Restaurants of Colorado, Inc., at 1905 S Federal Boulevard, Denver, CO: On and preceding 3/19/15 McDonald's Corporation did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from acids or caustic liquids.  Employees filled mop buckets with Kay Chemical Company SolidSense FloorCare A (containing 20-50% citric acid).  The corrosive liquid at dilution concentration has a pH of 4.5.  Employees did not use eye or face protection while filling the mop bucket.  This condition exposed the employees to a chemical hazard.

One employee filtered approximately 250 degree Fahrenheit oil while cleaning the fryer.  The employee was not using heat resistant gloves while filtering the oil.  This condition exposed the employee to a burn hazard.     (b) McDonald's Restaurants of Colorado, Inc., at 1905 S Federal Boulevard, Denver, CO: On and preceding 3/19/15 McDonald's Restaurants of Colorado, Inc. did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from chemical burns. The employer did not ensure that heat resistant gloves were available and worn when employees cleaned or scraped the grills.  This condition exposed the employees to a burn hazard.

29 CFR 1910.132(f)(3):  When the employer had reason to believe that an affected employee who had already been trained did not have the understanding and skill required by paragraph (f)(2) of this section, the employer did not retrain that employee:    (a) McDonald's Restaurants of Colorado, Inc., at 1905 S Federal Boulevard, Denver, CO: On 3/19/15 when McDonald's Restaurants of Colorado, Inc. had reason to believe that an affected employee who had already been trained did not have the understanding and skill required by paragraph (f)(2) of this section, the employer did not retrain that employee.  One employee filtered approximately 250 degree Fahrenheit oil while cleaning the fryer.  The employee was observed by management frequently not using heat resistant gloves, apron, or eye/face protection when performing this task.  The employer did not retrain the employee with regards to use of personal protective equipment when cleaning the fryer.  This condition exposed the employee to a burn hazard.
