29 CFR 1926.405(b)(1): Unused openings in cabinets, boxes, and fittings were not effectively closed: a) Embassy Security Group Facility, Quantico Marine Base: On or about 03/25/2015, the 240-volt breaker panel was not provided with a cover over the openings in the faceplate and bottom cabinet thus exposing employees to live bus bars and potential arc flash. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.405(b)(2): In energized installations, each outlet box did not have a cover, faceplate, or fixture canopy: a) Embassy Security Group Facility, Quantico Marine Base: On or about 03/25/2015, an energized 120-volt receptacle outlet was missing a faceplate thus exposing employees to live conductor wires. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
