29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  On or about 03/31/15 located at 4110 Cameron Ct., Cumming  , GA: Employees were framing a house with lumber and plywood while on the first and the second floor as joist and lumber were sung overhead as employees were working below, without hard hats exposing them to a struck by hazards.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   On or about 03/31/15 located at 4110 Cameron Ct., Cumming, GA:  Employees were observed installing plywood on to the roof of a house that was under construction with a pneumatic nail gun and using a skill saw without safety glasses.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about 03/31/15 located at 4110 Cameron Ct., Cumming, GA: Employees were framing a house that was under construction while installing plywood and rafters onto the roof while working at the edge, the employer did not ensure that fall protection was used by the employees while exposed to fall hazards of approximately up to 25 feet and the pitch was 8/12.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about 03/31/15 located at 4110 Cameron Ct., Cumming, GA: Employees were exposed to fall hazards, while performing framing work, employees were using a portable extension ladder to gain access to the 2nd floor, and the ladder was only 1 foot above the landing surface.
