29 CFR 1910.24(f): Rise height and tread width were not uniform throughout any flight of stairs including any foundation structure used as one or more treads of the stairs: a) Fuel Bulk Point, where contract workers were exposed to fall and tripping hazards while utilizing stairways including: 1. Tank 1, where the concrete cover for the stilling tank measured 24 inches above ground level. 2. Tank 1, where the rise between the concrete cover for the stilling tank and the metal stairway measured 13 inches. 3. Tank 2, where the concrete cover for the stilling tank measured 18 inches above ground level. 4. Tank 2, where the rise between the concrete cover for the stilling tank and the metal stairway measured 13 inches.

29 CFR 1910.24(g): Stairway platforms shall be no less than the width of a stairway and a minimum of 30 inches in length measured in the direction of travel: a) Fuel Bulk Point, where contract workers were exposed to fall and tripping hazards while performing job tasks on stairway platforms measuring less than 24 inches including: 1. Tank 1, where the width of the platform ranged from 15 ¾ to 16 inches due to the placement of the high level alarm canister. 2. Tank 2, where the width of the platform ranged from 14 to 18 inches due to the placement of the high level alarm canister. 3. Tank 3, where the width of the platform ranged from 15 3/4 to 16 inches due to the placement of the high level alarm canister.
