29 CFR 1910.134(f)(2):  The employer did not ensure that employees using tight fitting face piece respirators pass an appropriate qualitative or quantitative fit test prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter.     (a) On or about 03/31/2015, at facility, for the EMT wearing Cordova N95 respirator while responding to people with conditions such as, but not limited to, ebola, tuberculosis, and respiratory infections.  The employees were not provided a fit test prior to being required to wear the respirator.

29 CFR 1910.151(c):  Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    (a) On or about 03/31/2015, at facility, for the paramedic and EMT using corrosive bleach and Spartan TB-Cide Quat.  An eye wash station was not immediately available for use in the event of eye contact.

29 CFR 1910.1030(c)(1)(iv): The Exposure Control Plan was not reviewed and updated at least annually and whenever necessary to reflect new or modified tasks and procedures which affect occupational exposure and to reflect new or revised employee positions with occupational exposure:     a) On or about 03/31/2015, at facility, for employees with occupational exposure to potentially infectious material.  Employer did not review and update on an annual basis it's exposure control plan.  The plan has not been reviewed in over three years.
