29 CFR 1910.23(e)(1): Standard railing(s) did not consist of top rail, intermediate rail and/or posts and/or did not have a vertical height of 42 inches (106.7 cm) nominal from upper surface of top rail to floor, platform runway, or ramp level:      On 10/1/2014, in the production area, employees working on platforms were exposed to fall hazards over 5 feet. A single guardrail measured 33 inches above platform levels and there was no mid rail installed.

The employer used employees that already had experience driving forklifts. Prior to allowing the employees to operate their forklifts, the employer failed to train the operators or evaluate their previous experience.

29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft.    On 10/1/14, in the production area, employees were exposed to unguarded rotating shafts on the hopper conveyor. Shaft ends extended 3 inches and were 1 inch in diameter. The employer installed a yellow plastic chain to keep the employees out of the area.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed.    On or about 10/1/14,  employees working on the shaving mill are exposed to a caught by hazard, including, but not limited to  the following:    a. employees operating the log deck conveyor were exposed to being caught in the unguarded chain and sprocket that was approximately waist height and within 12 inches of where the employees are standing.    b. employees working on or near the conveyor, where chain and sprockets move the conveyor, employees are exposed to a caught in hazard by the unguarded chain and sprocket.

The logs fall to the lower level 5'-6" to the finished floor presenting a crushing hazard to the  employees working in the area.
