29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of Particulates not otherwise regulated (PNOR) listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 15 mg/m3:    An employee engaged in the grinding casting on July 1, 2015 was exposed to particulates not otherwise regulated (total dust) at an 8-hr time-weighted average level of 30.05 mg/m3, approximately 2 times the limit of 15 mg/m3; this limit established to prevent lung disease. The employee used a hand held grinder to accomplish the tasks. The exposure level is derived from one sample collected over a 344 minute period, assuming 0 exposure for the remaining 136 minutes.

29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:    Employees engaged in the grinding casting were exposed to continuous noise over the 8-hour time weighted average sound level of 85 decibels and did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o):      (a) An employee engaged in grinding activities was exposed to continuous noise at 420% of the permissible daily dose (8-hour time-weighted average sound level of 85 dBA) or an equivalent sound level of approximately 100.3 dBA during the 442 minute sampling period on April 7, 2015. Exposure calculations included a zero increment for the 38 minutes not sampled.      (b) An employee engaged in grinding activities was exposed to continuous noise at 225% of the permissible daily dose (8-hour time-weighted average sound level of 85 dBA) or an equivalent sound level of approximately 95.8 dBA during the 315 minute sampling period on April 7, 2015. Exposure calculations included a zero increment for the 165 minutes not sampled.      (c) An employee engaged in grinding activities was exposed to continuous noise at 203% of the permissible daily dose (8-hour time-weighted average sound level of 85 dBA) or an equivalent sound level of approximately 95.1 dBA during the 292 minute sampling period on April 7, 2015. Exposure calculations included a zero increment for the 188 minutes not sampled.      (d) An employee engaged in grinding activities was exposed to continuous noise at 149.5% of the permissible daily dose (8-hour time-weighted average sound level of 85 dBA) or an equivalent sound level of approximately 92.9 dBA during the 180 minute sampling period on April 7, 2015. Exposure calculations included a zero increment for the 300 minutes not sampled.
