29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.   (a) Insulation Technology Corporation @ 4040 Kodiak Court, Frederick CO 80504.  On and before March 31, 2015, the employer did not have a written Lockout/Tagout program.   Three employees work in the machine shop area, which contains a CNC machine.  Some elements of a Lockout/Tagout program were in place but the employer did not ensure that a written machine specific procedure had been developed and implemented for machines such as but not limited to the CNC.  This condition exposed employees to caught-in hazards associated with the unexpected start up of the CNC machine.

29 CFR 1910.1052(d)(1)(i): Where Methylene Chloride is present in the workplace, the employer shall determine each employee's exposure by either:  (A): Taking a personal breathing zone air sample of each employee's exposure; or  (B): Taking personal breathing zone air samples that are representative of each employee's exposure.  (a) Insulation Technology Corporation @ 4040 Kodiak Court, Frederick CO 80504.  On and before March 31, 2015, the employer did not perform employee exposure monitoring for methylene chloride. The employer had been using methylene chloride as a cleaning solvent.  This condition exposed employees to health hazards associated with exposure to methylene chloride.

29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.  (a) Insulation Technology Corporation @ 4040 Kodiak Court, Frederick CO 80504.  On and before March 31, 2015, the employer did not have written documentation of a Personal Protective Equipment (PPE) Hazard Assessment.    The employer had identified chemical hazards based on Safety Data Sheet (SDS) information and provided the correct types of PPE for work processes.  However, there was no written record of the PPE Hazard Assessment.  This condition potentially exposed employees to wearing incorrect or inadequate PPE.

29 CFR 1910.134(c)(2)(i): An employer may provide respirators at the request of employees or permit employees to use their own respirators, if the employer determines that such respirator use will not in itself create a hazard. If the employer determines that any voluntary respirator use is permissible, the employer shall provide the respirator users with the information contained in Appendix D of 29 CFR 1910.134.    (a) Insulation Technology Corporation @ 4040 Kodiak Court, Frederick CO 80504.  On and before March 31, 2015, the employer did not provide respiratory training for employees who were provided with a tight-fitting, half face air-purifying respirator.  This condition potentially exposed employees to respiratory hazards.

29 CFR 1910.1026(d)(1): General. Each employer who has a workplace or work operation covered by this section shall determine the 8-hour TWA exposure for each employee exposed to chromium (VI). This determination shall be made in accordance with either paragraph (d)(2) or paragraph (d)(3) of this section.  (a) Insulation Technology Corporation @ 4040 Kodiak Court, Frederick CO 80504.  On and before March 31, 2015, the employer did not perform employee exposure monitoring for stainless steel welding operations.  One employee conducts stainless steel welding approximately 3-4 times per quarter for 15 minutes at a time.  This condition potentially exposed employees to respiratory hazards associated with hexavalent chromium.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. On and before March 31, 2015, the employer did not provide Hazard Communication training which includes information on the Globally Harmonized System of Classification and Labeling of Chemicals (GHS).  This condition potentially exposed employees to chemical hazards.
