29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  Location: Jobsite  a) Employees were exposed to a fall hazard of up to 25 feet while working from the fifth level of the tubular welded frame scaffold where no guardrails or fall protection was used.  b) Employees were exposed to a fall hazard of approximately 23 feet while crawling across a 2' x 12' piece of framing lumber attached on one end to a roof bracket scaffold and on the other end to the tubular welded frame scaffold.  c) Employees were exposed to a fall hazard of up to 23 feet while working from a roof bracket scaffold with no fall protection.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces shall not be used as a means of access.    Location: Jobsite    Employees were exposed to fall hazards of greater than 10 feet, while climbing the frames of a tubular welded frame scaffold to their working levels, where safe access was not provided.

29 CFR 1926.454(a)(1): The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area.    Location: 227 North Central Street, East Bridgewater, MA.    Employees were exposed to fall hazards at heights between 23 and 25 feet, where Employees were not trained to recognize the fall hazards associated with the type of scaffolds being used.
