29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person    a) Rio Grande Insulation Inc. at 1000 Speer Blvd, Denver, CO 80204:  On or before April 14th, 2015 employees spraying and assisting in the application of spray foam insulation were required to wear North full face respirators and the employer did not ensure that frequent and regular inspections were being conducted by a competent person to identify hazards in the workplace.  During the inspection respirators were found unprotected from environmental contamination in the back of the job trailer where barrels of spray foam insulation components were kept.

29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form:   a) Rio Grande Insulation Inc. at 1000 Speer Blvd, Denver, CO 80204:  On or before April 14th, 2015 an employee applying spray foam insulation was exposed to potential hazards from exposure to methylene bisphenyl isocyanate (MDI) and the employer failed to sample and evaluate respiratory hazards in the workplace.

29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or  were not packed or stored to prevent deformation of the facepiece and exhalation valve:   a) Rio Grande Insulation Inc. at 1000 Speer Blvd, Denver, CO 80204:  On or before April 14th, 2015 employees spraying and assisting in the application of spray foam insulation were required to wear North full face respirators and the employer did not ensure that the employees respirators were stored appropriately to avoid environmental contamination.  This exposed the wearers to potential hazards in the event that the respirators became contaminated as they were found unprotected in the back of the job trailer where barrels of spray foam insulation components were kept.

29 CFR 1910.134(k)(1)(v): The employer did not ensure that each user could demonstrate knowledge of the procedures for respirator maintenance and storage:  a) Rio Grande Insulation Inc. at 1000 Speer Blvd, Denver, CO 80204:  On or before April 14th, 2015 employees spraying and assisting in the application of spray foam insulation were required to wear North full face respirators and the employer did not ensure that employees had received appropriate training and were knowledgeable on how to properly store respirators to prevent environmental contamination. This exposed the wearers to potential hazards in the event that the respirators became contaminated as they were found unprotected in the back of the job trailer where barrels of spray foam insulation components were kept.
