29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet (1.22 m) or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides: (a) Employees accessing the top of molds to perform setting of cores, mold washing and torching of the mold were exposed to a fall hazard greater than four (4) feet.

29 CFR 1910.179(n)(3)(iii)(b): During hoisting, the load contacted obstructions: (a) Overhead Crane No. 1: A ladle containing molten steel and being handled by the overhead crane operator was not verified to have been clear of the mobile pouring staircase. As the ladle was hoisted and moved, the ladle handle caught under the top rail of the staircase causing the stopper rod to open and release molten steel onto the mold and pouring floor. Employees were exposed to a burn hazard.
