A). on or about 04/01/15, at the site of 82-56 212nd Street. Queens Village, NY Soil & tools were placed by the edge of the excavation 35 ft. x 2 ft. x 6 1/2 ft. in depth. Three employees working in an unprotected excavation were exposed to the hazard of cave-in. ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.
