At the worksite located 3757 Itaska Street, Saint Louis, Missouri, employee(s) for Armando Rivas De La Cruz, were observed working below the residential roof where roofing materials were being tossed to the ground, without the use of protective helmets.

At the worksite located 3757 Itaska Street, Saint Louis, Missouri employee(s) for Armando Rivas De La Cruz were observed installing shingles on a residential roof using pneumatic nail guns without the use of eye protection.

Employee(s) for Armando Rivas de la Cruz were observed installing shingles on the roof of the residence without fall protection.  The fall distance to the ground was approximately 10 feet (ladder rungs).  2.  Employee(s) for Armando Rivas de Cruz were observed installing shingles on the roof of the garage adjacent to the residence without fall protection.  The fall distance to the ground was approximately 10 feet (ladder rungs).
