29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:    On or about April 1, 2015 and times prior thereto, at the worksite located at or near 3007 S Olive Street, Pine Bluff, Arkansas 71602, the exit door located by the area marked sneakers and $14.99 products was marked as an exit but was padlocked from the outside. This condition exposes employees to fire hazards.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):    On or about April 1, 2015 and times prior thereto, at the worksite located at or near 3007 S Olive Street, Pine Bluff, Arkansas, the duplex outlet located near the cash register was not guarded against accidental contact and had live, energized parts exposed. The duplex had a printer and unidentified green extension cord plugged into it. This condition exposed employees to shock hazards.

29 CFR 1910.304(b)(2)(iii): The grounding contacts of receptacles and cord connectors were not grounded by connection to the equipment grounding conductor of the circuit supplying the receptacle or cord connector. The branch circuit wiring method did not included or provided an equipment grounding conductor to which the grounding contacts of the receptacle or cord connector were not connected.    On or about April 1, 2015 and times prior thereto, at the worksite located at or near 3007 S Olive Street, Pine Bluff, Arkansas, the duplex outlet that had the orange extension cord hard-wired by the cashier's area was not grounded. The duplex had a printer and unidentified green extension cord plugged into it. This condition exposed employees to shock hazards.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:    On or about April 1, 2015 and times prior thereto, at the worksite located at or near 3007 S Olive Street, Pine Bluff, Arkansas, extension cords were observed being used as permanent wiring. The cords were not available for further inspection. The ceiling in the facility had visible signs of water leaks, exposing the cords to wet conditions. This condition exposed employee(s) to fire hazards and shock hazards. This condition was observed in the following instances:    Instance A: A green extension cord attached to the wall located to the right of the platform at the front of the store. It disappeared into the ceiling.    Instance B: A white extension cord attached to the wall located to the right of the platform at the front of the store. It disappeared into the ceiling.

29 CFR 1910.305(g)(1)(iv)B: Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used to run through holes in walls, ceilings, or floors.    On or about April 1, 2015 and times prior thereto, at the worksite located at or near 3007 S Olive Street, Pine Bluff, Arkansas, extension cords were observed being used in an unpermitted manner. The extension cords were being used through a hole in the ceiling tile up front by the cash register area. The cords were not available for further inspection. The ceiling in the facility had visible signs of water leaks, exposing the cord to wet conditions. This condition exposed employee(s) to fire hazards and shock hazards. This condition was observed in the following instances:    Instance A: A green extension cord attached to the wall located to the right of the platform at the front of the store. It disappeared into the ceiling.    Instance B: A white extension cord attached to the wall located to the right of the platform at the front of the store. It disappeared into the ceiling.
