29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:  The employer failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1 Item 1-b for hazards associated with falls and providing fall protection training to each employee exposed to fall hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about March 31, 2015 the employer, Randall Exterior Construction LLC. located at 5508 Superior Ave., Cleveland OH did not ensure that their 3 man crew working at a residential site located at 3906 Memphis Ave., Cleveland OH were protected from a fall height of approximately 15 feet 10 inches by utilizing fall protective systems/devices while performing weather proofing on a 12/12 pitch roof.
