29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a. Arbor Chase Subdivision, 2940 Arbor Vine Way, Lot #44, Cumming, GA (Jobsite) - Employees were exposed to fall hazard. The employee was not utilizing a means of fall protection while working and walking approximately 24 feet above ground level on the elevated makeshift 2x10 plywood platform, while installing brick to the newly constructed two-story residential home, on or about 04/01/15.

29 CFR 1926.702(i)(1): Masonry saws were not guarded with a semicircular enclosure over the blade: a. Arbor Chase Subdivision, 2940 Arbor Vine Way, Lot #44, Cumming, GA (Jobsite) - An employee was exposed to struck-by hazard. Employee was operating the portable masonry saw without having the guard covering the rotating part/blade. An employee was observed cutting bricks having the saw guard tied back with a string to the saw frame, exposing the rotating blade, on or about 04/01/15.
