29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Jobsite, 2413 High Oak Road, Pittsburgh, PA: On or about April 1, 2015, two employees were installing shingles on a residential roof and were not protected from falling. The fall hazard for the one employee was approximately 20 feet to the driveway below.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards: a) Jobsite, 2413 High Oak Road, Pittsburgh, PA: On or about April 1, 2015, two employees were working on a residential roof over 6 feet and had not been trained to recognize or protect themselves from the hazards of working at heights.
