OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation hazards:     (a)Bos Solutions, Inc. @ Watson B Ranch 32-C, Rig #7, Battlement Mesa, CO 81635: On and before March 14th, 2015, the employer did not ensure that each employee performing unjamming activities associated with the MMG-001, Cutting Particle Sizer Machine, were protected from amputation hazards. The MMG-001 Cutting Particle Sizer Machine jammed on a frequent basis due to the heavy flow of large-sized rocks and gravel from the surface of the drilling site.  On March 14th, 2015, during the night shift, a field technician reached inside the cutting inlet of the machine to remove a rock without de-energizing and/or shutting off the machine to prevent unexpected start up resulting in a left hand amputation.  The condition exposed employees to amputation hazards.    Abatement Note: Among other methods, one feasible and acceptable method to correct this hazard is to develop and implement specific procedural elements to control hazardous energy in accordance with the American National Standards Institute (ANSI)-American Society of Safety Engineers (ASSE) Z244.1-2003 publication.

The branch circuits located within electrical panel #1 which was rated at 480V 60 amp 3 phase, were not legibly marked to indicate its purpose. This condition exposed employees to electrical and amputations hazards.
