29 CFR 1926.651(b)(3): When excavation operations approached the estimated location of underground installations, the exact location of the installations was not determined by safe and acceptable means: (a) On or about March 10, 2015 at the jobsite north of Honeysuckle on County Rd A, Appleton, underground utilities were not adequately located where the drilling head traveled through the soil. A pothole was not made around the entire utility prior to drilling.
