29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.    a. Maintenance Shop - On or about April 1, 2015, supplies and other material were stored haphazardly. The maintenance shop was clutter with boxes, paints and material.  Oxygen cylinders and unknown compressed gas cylinders were stored on a bottom shelf behind the refrigerator.  Oxygen cylinders should not be stored with other compressed gases and petroleum products such as paint, oil or fuels.

Apartment B-2 - On or about April 1, 2015, the grounding wire that connects to the top of the water heater was not connected.  A grounding wire should have been connected at the location where the electrical wiring enters the hot water heater.

Maintenance - On or about April 1, 2015, the maintenance employees were voluntarily wearing air-purifying respirators when spraying appliance epoxy on counter tops.  The employer has not developed and implemented a respiratory protection program for voluntarily use of a respirator.

Apartments D-3 and U-2 - On or about April 1, 2015, the electrical wiring for the hot water heater in apartment D-3, electrical wiring under the sink in apartment D-3 and the electrical wiring for the garbage disposal in apartment U-2 was spliced and capped, but the splice was not in an approved enclosure.
