29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) 1823 Salem Road, Wyomissing, PA; Employees were installing drip edge 8 feet 3 inches above the ground without the use of fall protection, on or about 04/02/2015.  Skyline Roofing & Construction LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 948495, citation number 1, item number 2 and was affirmed as a final order on 12/30/2015, with respect to a workplace located at 22 Thomas Street, Cressona, PA 17929.     Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   a) 1823 Salem Road, Wyomissing, PA; Employees used a Werner 22 foot Extension Ladder that was not placed 3 feet above the landing in order to access the roof work area, on or about 04/02/2015.  Skyline Roofing & Construction LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 948495, citation number 1, item number 3 and was affirmed as a final order on 12/30/2015, with respect to a workplace located at 22 Thomas Street, Cressona, PA 17929.
