29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On March 31, 2015 and at times prior thereto; second level; employees framing and sheathing the remodeled residential home were observed sitting or working off the rafters without fall protection in place to prevent them from falling 35 feet to ground level. b) On March 31, 2015 and at times prior thereto; second floor area; employees were exposed to unguarded open floors exposing to a 10 foot fall to the lower level.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge: a) On March 31, 2015 and at times prior thereto; second floor; the stairs used to access the upper floors were not properly guarded exposing employees to a 54 inch fall to the lower platform.
