29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous. On or about April 2, 2015 and at times prior thereto, employees were exposed to electrical hazards because the grounding pin had been removed from an electrical extension cord that was attached to a 6 gallon porter-cable air compressor. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date completed, method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.405(g)(2)(iii): Flexible cords were not used in continuous lengths without splice or tap. On or about April 2, 2015 and at times prior thereto, employees were exposed to electrical hazards because the power cord of a 5 gallon porter-cable air compressor was spliced with an electrical extension cord covered with electrical tape. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date completed, method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On or about April 2, 2015 and at times prior thereto employees were exposed to fall hazards while installing solar panels on the two story residence without any fall protection, exposing the employees to a fall distance of approximately 19' to the ground. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
