29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access: a) 319 Bliss Rd., Longmeadow, MA: On or about March 31, 2015, several employees were observed using ladders that did not extend the required 3 feet past the upper landing surface (roof), to access said roof of this residence they were in the process of shingling. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system: a) 319 Bliss Rd., Longmeadow, MA: On or about March 31,2015 several employees were roofing this residence without any fall protection between 11 and 20 feet above the ground below. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1). AJG Construction, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501 B (13), which was contained in OSHA inspection number 756641, citation number 1, item number 1 and was affirmed as a final order on February 15, 2013, with respect to a workplace located at 1657 Northampton St., Holyoke, MA.
