29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources:       On or about April 2, 2015, and at times prior thereto, at the establishment the employer did not provide lock-out/tag-out equipment to employees who may be required to perform lock-out/tag-out of energy sources.  The employees exposed to hazards caused by unexpected start-up of machines or equipment.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:         On or about April 2, 2015, and at times prior thereto, employees were allowed to operate machines that were found not having proper guards provided, including but not limited to the following instances:       1) At the 97 Ward Machine where the scrap belt continued to operate when the gates were opened.       2) At the single phacer machine, rollers were in operation without tripwires provided to shut the rollers off in case of emergency.       Employees were exposed to possible amputation and crushing hazards.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:       On or about April 2, 2015, and at times prior thereto, in the maintenance shop oxygen and acetylene cylinders were found stored together without a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour.  Employees were exposed to the hazard of fire.
