29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    a) At a customer job site: On or about March 24, 2015, the employer did not ensure a power washing employee used appropriate protective gloves, neoprene or PVC, while applying a hydrofluoric acid-containing solution of CM-X and water which resulted in the employee receiving chemical burns to his hands.      b) In the chemical dispensing area at 224 Cold Spring Avenue West Springfield, MA: On or about April 7, 2015, the employer did not ensure an employee used appropriate protective gloves, neoprene or PVC, while exposed to a hydrofluoric acid-containing cleaning solution, CM-X, during a chemical transfer from a 55 gallon drum to a 5 gallon container.
