29 CFR 1910.119(e)(3)(v): The process hazard analysis(PHA) did not address the hazards related to facility siting: At the time of the inspection, the employer had not addressed the hazards related to facility siting for the Ammonia Storage or Acid Plant PHAs. The employer continued to rely on statements that a comprehensive facility siting study was in progress. This in progress study had been referenced as far back as 12/27/2010. The Dyno Nobel company was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 314110578, citation number 1, item number 2a and was affirmed as a final order on 1/16/2012, with respect to a workplace located at Donora PA.
