29 CFR 1910.26(c)(3)(iii): The ladder base section must be placed with a secure footing. On or about March 26, 2015, a fiberglass extension ladder was used on a wet and slippery deck surface and the ladder was not stabilized or secured to prevent the ladder from being displaced. Among other methods, one feasible and acceptable abatement method to correct this hazard is to comply with the American National Standards Institute A14.5-1982 Section 9.3.4 Footing Support. Ladders shall not be used on ice, snow, or slippery surfaces unless suitable means to prevent slipping are employed.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toe boards: On or about March 26, 2015, employees were working from a residential roof cleaning gutter with no railings in place exposing them to a fall to the lower level. ABATEMENT CERTIFICATION IS REQUIRED FOR THIS ITEM IN ACCORDANCE WITH REQUIREMENTS OF 29 CFR 1903.19(C).
