OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that, employees were exposed to struck by and/or caught in machinery:   (a)Sterling Component Systems @ 626 South 11th Ave., Sterling, CO. 80751: On and prior to March 25th, 2015, the employer did not ensure that each employee working in the Roof Truss Assembly Area was protected from struck-by/caught-in-between hazards while working on or near the restricted zone when the Roof Truss Roller Press, Mitek II, RoofGlider was in operation. As a result, on March 25th, 2015, an employees lower left leg was amputated after getting caught between the truss press and truss table. The condition exposed employees to struck-by/caught-in-between hazards.   Abatement Note: Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow the manufacturers operation and maintenance manual. Develop and implement a written program to include training for all employees regarding safe operating procedures as listed in the manufacturers operations and maintenance manual. Elements should include but not be limited to establishing a "restricted zone" with "safe working boundaries", inspecting all emergency safety bars, stop control devices, warning horn/beacon to alert employees prior to motion of the machine, and a method to ensure all personnel are clear of operating machinery at all times. This instruction should includes, but not limited to;  (1) inspect the machine operations including the emergency safety bars and emergency stop controls and ensure that the warning horn and beacon is functioning to alert employees the machine is in motion; (2) become familiar with the warnings, such as before turning on the machine, (3) Make sure that all personnel and equipment are clear;  (4) Never stand in an aisle while the machine is in operation; "Never operate the RoofGlider without all safety guards in place and operational."
