29 CFR 1926.451(b)(1):   Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):    Work site located at 420 Steinhart Park Road, Nebraska City, NE:  The working level of a scaffold was not fully planked which decreases the working area for the employees.  On March 30, 2015, an employee fell from the working platform approximately 11.3 feet to the ground below due to a combination of issues with the scaffold which included incomplete decking.        Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.

29 CFR 1926.451(c)(2):   Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:  Work site located at 420 Steinhart Park Road, Nebraska City, NE:  A fabricated frame scaffold located at the site did not have mud sills installed at each lower frame location or other firm foundation.  This increases the potential for shifting of the scaffold while in use and possible collapse.      Abatement certification is required for this violation. Work site located at 420 Steinhart Park Road, Nebraska City, NE:  The employer failed to ensure that a complete guardrail system was installed on a platform prior to allowing employees to work.  On March 30, 2015, an employee fell from the working platform approximately 11.3 feet to the ground below due to a combination of issues with the scaffold which included the lack of guardrails.      Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. 29 CFR 1926.452(c)(2):   All cross braces were not of such length so as to automatically square and align vertical members such that the erected scaffold was always plumb, level, and square:    Work site located at 420 Steinhart Park Road, Nebraska City, NE:  The frames of the scaffold were not all connected as required using cross bracing to ensure that the scaffold would remain square and plumb.  The bracing utilized at the time of the inspection was incomplete and did not ensure that each scaffold frame was level, plumb, and square. Work site located at 420 Steinhart Park Road, Nebraska City, NE as well as throughout the company:  The employer has not provided training to each employee who was involved in erecting or disassembling scaffolding systems.  On March 30, 2015, an employee fell from the working platform approximately 11.3 feet to the ground below due to a combination of issues with the scaffold which were created due to a lack of knowledge on the correct erection of a fabricated frame scaffold.          Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. This should include training on mitigating those hazards through an acceptable method of fall protection.  Training should also include the requirements for fall protection systems and when it is appropriate to utilize those systems.
