29 CFR 1904.39(a)(2) - The employer did not report within 24-hours a work-related incident resulting in an in-patient hospitalization, an amputation or the loss of an eye: a) Facility, S.R. 191 & Chipperfield Drive, Stroudsburg, PA - An employee sustained a work-related injury and was admitted into the hospital. The in-patient hospitalization was not reported by the employer to OSHA within twenty-four (24) hours, on or about March 30, 2015. In accordance with 29 CFR 1903.19(c), abatement certification is not required for this violation. The employer notified OSHA during the inspection.

29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes. a) Battery Room, S.R. 191 & Chipperfield Drive, Stroudsburg, PA - The employer provided and required the use of latex gloves by employees instead of butyl or nitrile gloves when inspecting and replacing water in powered industrial vehicle batteries, on or about April 3, 2015. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.
