29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program:  (a) Coconino National Forest: On or about 31 March 2015 and at times prior thereto, the agency did not develop and implement a monitoring program for noise exposure. Employees are exposed to high noise levels generated by gas-powered tools and heavy equipment. In addition, Law Enforcement Officers are exposed to high impact noise levels during firearm qualifications.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a) Archaeology Lab: On or about 31 March 2015 and at times prior thereto, power strips (relocatable power taps) were daisy-chained and plugged into extension cords. Per the UL White Book, a) Relocatable power taps are intended to be directly connected to a permanently installed branch circuit receptacle and, b) Relocatable power taps are not intended to be series connected (daisy chained) to other relocatable power taps or to extension cords.   The U.S. Forest Service was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(2), which was contained in OSHA inspection number 982198, citation number 2, item number 3 and was affirmed as a final order on December 19, 2014, with respect to a workplace located at 29835 Valyermo Road, Valyermo, CA 93563.  The U.S. Forest Service was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(2), which was contained in OSHA inspection number 957451, citation number 2, item number 5, and was affirmed as a final order on April 7, 2014, with respect to a workplace located at 1634 Black Canyon Rd., Goose Valley Fire Station, Ramona, CA 92065.
