29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: 1) Production Area, Press #98 - Employees were operating a printing press machine M98 model M1000BE and serial OR2800 that was not adequately guarded on all parts of the machines. An employee was injured while conducting webbing operation in crawl mode approximately 28 feet per minute. Employees were exposed to an amputation hazard. The Quad Graphics, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 314662966, citation number 01, item number 001 and was affirmed as a final order on May 26, 2011, with respect to a workplace located at 380 W. 37th Street, Loveland, CO 80538.
