29 CFR 1926.602(c)(1)(ii): Modifications or additions affecting the safe operation of the equipment were made without the manufacturer's written approval: (a) At the Lighthouse of Howard jobsite; a Star Industries Heavy Duty Jib (Model: 1360B) was added to the Terex Handlers (Model: SS8420, S/N: 014031) without having the nameplate changed to reflect the de-rated capacity. On or about March 31, 2015, two bundles of framed walls (bundle 11 and 55) were lifted using the Terex Handler and Star Industries Heavy Duty Jib. This modification de rated the equipment lifting capacity and while the bundles of materials were being moved, the back tires of the Terex Handler left the ground. This exposed employees to struck-by, equipment tipping and rollover hazards.
