29 CFR 1926.501(b)(1):  Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:   A.)  Visual Communications, Inc. dba Viscom Systems, 101 East 6th Street, Erie, PA  16501, mezzanine area, on or about March 31, 2015 and times prior there to:  An employee was performing work activities in the mezzanine area that included, but was not limited to, installing security cabling.  The employee was exposed to a fall hazard of 8.6 feet.  The employee not utilizing a means or method of fall protection including, but not limited to, an adequate guardrail system, safety net system, or personal fall arrest systems.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:  A.)  Visual Communications, Inc. dba Viscom Systems, 101 East 6th Street, Erie, PA  16501, mezzanine area, on or about March 31, 2015 and times prior there to:  Employees were performing work activities including, but not limited to, installing security cabling in the mezzanine area.  Access to the mezzanine area required the employees to step up from a lower level to an upper level.  The break in elevation bewteen the lower level and upper level was 28.5-inches in height.  Ladders, stairs or other suitable means were not provided when traveling between the two levels.
