29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    OR IN THE ALTERNATIVE    29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:    a) Throughout Plate Department, Rear of Plate Machines - Employees were not protected from being caught in the cutting dies and/or kick-out plate if they released while performing tasks such as, but not limited to, clearing paper jams, thus exposing employees to crushing and/or amputation hazards.      b) Throughout Plate Department, Sides of Plate Machines - Employees were exposed to crushing and/or amputation hazards because they were not protected from being caught in the cutting dies and where scrap paper exited on the sides of the machines, as listed below:    The sides of machines #1 through #4 and #11 through #17 did not have interlocking mechanisms on the cutting die guards.    The cutting dies on the sides of machines #5 through #10 were not guarded.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:    a) Plate Department, Plate Machine #9 - On or about April 2, 2105, there was an unused opening on the electrical box that controlled the belt motor on machine #9, thus exposing employees to electrical hazards.      AJM Packaging Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.305(b)(1)(ii), which was contained in OSHA inspection number 314094186 , citation number 1, item number 2 and was affirmed as a final order on October 5, 2011 with respect to a workplace located at 100 Sack Drive, Folkston, GA 31537.
