29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) The John Deer 6310 tractor's PTO was missing the shaft cover, exposing employees to rotating parts.

29 CFR 1910.305(a)(2)(ix): Lamps for general illumination were not protected from accidental contact or breakage by a suitable fixture or lampholder with a guard. (a) The Pump House light bulb, located 86" from the floor, was not protected from damage. (b) The light bulb located in the Fern Ridge Dam's electrical control room was not protected from damage. The Army Corps of Engineers was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1910. 305(a)(2)(ix), which was contained in OSHA Inspection Number 941517, Citation Number 2, Item Number 5 , and was affirmed as a final order on 12 /11/2013 with respect to a work place located at, Rufus, OR and Inspection Number 96892 Citation 2, Item Number 2, and was affirmed as a final order on 09/16/2011. Note: Abatement certification and supporting documentation are required for these items.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: Pump House- (a) The Flotec power cord was not connected to the unit to protect the conductors from damage. The Army Corps of Engineers was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1910. 305(g)(2)(iii), which was contained in OSHA Inspection Number 941517, Citation Number 2, Item Number 6 , and was affirmed as a final order on 12 /11/2013 with respect to a work place located at, Rufus, OR.
